by physician referral only. A prescription/ referral
form may be downloaded below. Patients in need
of treatment not prescribed by a physician are
eligible for treatment with Penny Nelson.

accept most major medical insurances including Medicare,
Blue Cross/Blue Shield, PPOM, Worker’s Comp.
and Auto. Physicians must prescribe services in order
for patients to be eligible for insurance-approved
visits. However, patients in need of treatment and
not prescribed by a physician, are eligible for treatment
with Penny Nelson.
